Payment Policies
We are in network with AETNA however, a patient responsibility often applies. During a consultation unless it is for mother only, normally both mother and baby are cared for therefore, both mother and baby are billed. The reimbursement rates can vary widely according to your AETNA plan type. Generally an estimated OOP patent responsibility will vary anywhere from 0-$100 and it can change according to your number of visits.
For other insurances (certain BCBS, United Healthcare or ANTHEM Plans) we currently partner with The Lactation Network . In order to verify your coverage through The Lactation Network follow the link above. All other payments will be made at time of service via credit card on file. We will provide you with a superbill suitable for you to submit to your insurance. The superbill (which will also serve as a payment receipt) will be coded appropriately to the level of service provided during the visit.
*For Lactation, our home visit service area is within a 25 mile radius of our office ( Columbia, SC).
All Home Visits Will be charged a convenience fee not covered by insurance-
$75 within 25 miles of our office.
$125 25-50 miles of our office.
$200 50-100 miles of our office.
This fee is not reimbursable from insurance. Bornwell accepts payments at the time that services are provided. .
All services are non-refundable.
A surcharge will be applied if you have twins or higher order multiples to cover the extra length of the visit.
These surcharges are not eligible for insurance reimbursement.
Method of Payment: Payments may be made electronically using a credit card (HSA/FSA eligible). We use Square to process payments. Square meets the high standards of HIPAA and the banking industry for security and privacy with regard to financial transactions.
Cancellation policy: Client is responsible for all charges associated with the visit. If client cancels with less than 24 hours notice, the credit card on file will be charged a late cancellation fee in the amount of 100% of service cost.
If Bornwell must cancel an appointment for any reason, no charges to the client will apply. Bornwell reserves the right to cancel appointments for any reason without notice.

Reimbursement Options For Prenatal Classes:
1) Many health insurance companies offer reimbursement for all or part of prenatal classes including Childbirth Education, Breastfeeding, Newborn Care, and Infant CPR, safety classes and lactation consultations. You must contact your own health insurance company to confirm your policy’s specific reimbursement benefits.
2) Flexible Spending Account (FSA)/ Health Care Savings Account (HSA) may be used to pay for all or any remaining balance of prenatal classes. Contact your own FSA/HSA to inquire about specific reimbursement policies. To learn more about Flexible Spending Accounts reimbursement, see the www.IRS.gov page.

Individual health insurance policies and reimbursements vary greatly. Please contact your health insurance company to confirm your benefits for reimbursement. Here is some additional information your health insurance company may ask you to provide.
Proof of Class Payment: After you sign up for a class you will receive an email with that contains confirmation of class registration and payment amount. This email can be used as a receipt of class payment. A copy of your credit card statement can also be used as proof of class payment.
Proof of Class Attendance: Once you have attended class you will be given a form that confirms class completion. This will include the date, duration and description of the class. You can submit this for insurance reimbursement.
